Notice of Agency Relationship Form (West Virginia)

West Virginia requires agency disclosure on the Commission's prescribed notice of agency relationship and consent form, showing whether each licensee represents the seller, the buyer, or both. The licensee and client sign it when the agency relationship is created, and all parties sign each licensee's form before negotiations begin. The broker gives copies to signers and keeps a signed copy in the file. Team members must all be listed. The form discloses the relationship but is not the listing or buyer agency agreement.
